Reflection on End of Life Choices
In my journey through the critical care environment, I've developed a deep passion for dignified deaths and the importance of our choices in approaching our own mortality.
I've experienced moments of profound emotion, from tears shed while performing CPR as family members demanded it, to the joy of bringing individuals back from acute events. I've witnessed both beautiful passings and ones that felt far from what anyone would have wanted.
One tool I believe is essential in honoring our end-of-life wishes is the POLST (Physician Orders for Life-Sustaining Treatment) form. It's a document that allows us to outline our preferences for medical care and designate a trusted medical power of attorney. This person is tasked with upholding our choices, especially in situations where loved ones may struggle to let go.
Too often, I've seen individuals encounter the POLST form for the first time in the chaos of an emergency room, overwhelmed with fear and uncertainty. Others encounter it in doctor's offices, sometimes without proper understanding or guidance.
That's why I've made it my mission to become certified with the Coalition for Compassionate Care of California, providing education and workshops on the POLST form. I believe that being proactive in our end-of-life choices is a gift to ourselves and our loved ones.
I offer consultations and workshops to expand understanding of the POLST form, which is available in all states or in the process of adoption. While forms may vary, the underlying importance remains the same.
